Note: CORE Sports Medicine & Lifestyle Medicine is a direct pay practice. We do not accept insurance but you will be provided an itemized bill at the end of your visit for which you can submit to your insurance for reimbursement if you have out of network benefits. Payment will be due at time of visit. For bloodwork, medication and imaging tests, you will still be able to use your insurance benefits. Forms of payment accepted include cash, most major credit cards and HSA/FSA cards.
